- Mandatory Information : Screening Type, Residency, Registered patient, Last name, Given name, Gender, Date of birth, Nationality, E-mail, Contact number (Korea / Overseas, at least one), Address (Address in Korea / Overseas residential address, at least one), Own a Korean Resident Card, Overseas insurance (insurance company), Purpose of screening (e.g., symptoms, medical history, family history, diagnosis), Surgical and treatment history, Preferred screening date, Details of requested examination

- Optional Information : Patient ID (chart number), Middle name, Current medications, Available languages, Date of entry into Korea, Date of departure, Expected length of stay (days)

3. Retention and Use Period of Personal Information

- In accordance with the Medical Service Act

You may refuse to consent to the collection and use of personal information. However, refusal may result in restricted access to services.
