- Mandatory Information : Residency, Registered patient, Last name, Given name, Gender, Date of birth, Nationality, E-mail, Contact number (Korea / Overseas, at least one), Address (Address in Korea / Overseas residential address, at least one), Own a Korean Resident Card, Overseas insurance (insurance company), Symptoms and reason for visit, Preferred appointment date & additional notes

- Optional Information : Patient ID (chart number), Middle name, Available languages, Date of entry into Korea, Date of departure, Expected length of stay (days)

3. Retention and Use Period of Personal Information

- In accordance with the Medical Service Act

You may refuse to consent to the collection and use of personal information. However, refusal may result in restricted access to services.
